Please join us in a Zoom virtual classroom to learn about root cause analysis systems for incident investigation. Professionals at all levels of experience will benefit from one or both of these interactive training opportunities.

The Basic course August 25, 2021 provides a basic introduction to the root cause analysis system for incident investigation.

The Leaders course on September 15-16, 2021 provides a detailed description of a root cause analysis system for incident investigation.

All are welcome to register for either the Basic or Leaders course or both. There are no prerequisites for either course.

Root Cause Analysis System - BASIC

August 25, 2021  8:30am-4:00pm Pacific Time

One day course in a virtual classroom setting via real-time, two-way video conferencing

Registration fee: $195

This course provides a basic introduction to the root cause analysis system for incident investigation. Course participants will leave with a basic understanding of the procedure for incident investigation using root cause analysis and an introductory understanding of appropriate preventive measures or corrective actions.

Audience: All health and safety professionals and workers with an interest in safety and accident investigation. Course is open to all regions.

Learning Objectives:

Upon completion of this course, participants will be able to:

  1. Define root cause analysis
  2. Describe the process used for incident investigation
  3. Determine root cause from information collected in an investigation
  4. List possible corrective actions for prevention following an incident

Root Cause Analysis System - for LEADERS

September 15-16, 2021  8:30am-4:00pm Pacific Time

Two day course in a virtual classroom setting via real-time, two-way video conferencing

Registration fee: $390

This course provides a detailed description of a root cause analysis system for incident investigation. Course participants will leave with data collection and analysis skills that will equip them to conduct incident investigations using root cause analysis and to implement appropriate corrective actions.

Audience: Safety professionals, supervisors, and leads responsible for workplace safety and incident investigation. Course is open to all regions. The Basic course is not a prerequisite for the Leaders course.

Learning Objectives:

Upon completion of this course, participants will be able to:

  1. Conduct an incident investigation using root cause analysis
  2. Use appropriate interviewing techniques
  3. Collect, organize, and analyze data from a root cause analysis
  4. Implement corrective actions for prevention following an incident

Rocco Meraglia has over 35 years’ experience in health, safety, and emergency services implementation on major global projects. Rocco specializes in root cause analysis, risk management development, and team building workshops in courageous self- leadership, Normalization of Deviance Incident & Systems Evaluations and project management improvements.

Rocco is the Principle of MEM HSE Services Consulting, a business consulting firm with offices in Vancouver, British Columbia, and Seattle, Washington (Cashman Global). Rocco has worked on some of the largest projects in the world including Ground Zero, Katrina Relief efforts, Mega Mining Construction Projects (Ekati Diamond Mine, Diavik Diamond Mine, Voisey Bay Nickel, Baffinland, Snap Lake Diamond Mine, Panama’s Mina de Cobre, Ambatovy Nickel-Madagascar, Baro Alto Project, numerous Oil/Gas & Power projects as the field and corporate HSSE Director and VP of HSE for multiple mining and EPCM organizations. Rocco has presented at international conferences on the topics of Leadership, Teamwork, Root Cause Analysis, Hazard/Risk Management, Wellness Programs, and Emergency Preparedness.

Rocco can provide risk assessment & gap analysis of HSSE systems from actual activities to provide a value-added look at what the significant risks are and to close them utilizing the “Reason Model” focusing on defenses and organization systems. Rocco has conducted over 500 high potential root cause analysis incident investigations for clients globally.

Rocco’s current focus is on conducting objective and independent HS systems auditing, 3rd party Incident Investigation reviews or facilitation, and guest speaking on Leadership in Safety, site safety gap analysis evaluations and safety culture improvement strategies.
